Home At Last Program (HAL)
The Home At Last (HAL) program supports adults who are aging or have special
needs and would benefit from assistance in getting settled at home after a stay in hospital. This transition service is provided at no cost by trained and qualified
Personal Support Workers (PSW) who arrive at the hospital and drive clients
home. The PSW can fulfill client needs like grocery shopping, medication pick
up at the pharmacy, preparing a small meal and providing personal care and/or
homemaking services. In certain situations, clients are able to get home on their
own but have the HAL settlement service meet them there to provide the
supports necessary to ensure their care and safety.
Follow up is an important component of the HAL program. Every client receives
a follow up call once they have settled in at home and visits are arranged to
check on the client’s well being if needed. The HAL service also includes making
referrals to other community support services when beneficial to the client.
Clients can be brought home from hospitals either within or outside of Durham Region.
This hospital discharge and follow up service is offered to those living in Durham Region.
